Claim for adjustment of income tax – corporate entities (3309e)

This form is for corporate entities seeking to claim adjustment of their income tax assessment. Corporate entities include limited liability companies, associations and foundations, for example. 

You can file a claim for adjustment in MyTax, on a paper form, or by submitting a free-form request on paper. If you use the paper form, you must include with the form a clear written account of your claim and the grounds for the claim. 

The financial statement documents attached to the claim for adjustment to income taxes will not be forwarded to PRH.

Claim for adjustment in MyTax

If you file the claim for adjustment with the paper form, check the return address on the first page of the form. Always include with the form your claim and the grounds for the claim, as well as any other required enclosure forms, such as a power of attorney. 

If you submit a free-form claim for adjustment, make sure to state

  • the name and Business ID of the appellant company
  • reference to the tax year and to the decision appealed
  • how you want the taxes to be changed, and
  • what the grounds for your claim are.

Enclose any other supporting documents which you have not previously delivered to the Tax Administration.

The party signing the claim for adjustment must have the right to sign for the corporate entity. If necessary, include a power of attorney with the claim. 

Enclose a corrected financial statement with a corrected tax return.
