Affirmation given by U.S. pension fund (6160) Forms

Affirmation form designed for a U.S. pension fund, to be completed if dividend income sourced in Finland is paid to the pension fund. The form should be enclosed with the request for refund of withheld taxes.  Reference is made to the bilateral U.S.-Finnish tax treaty, provision of refunds under Article 10, Chapter 3.
